What to Bring to Your DOT Physical

To help your DOT physical exam go smoothly, please bring the following items to your appointment:

Required

Valid government-issued photo ID (driver’s license preferred)
Corrective lenses or hearing aids, if you use them for driving
List of current medications, including dosage and prescribing provider
Recent blood work, if any
Medical records or clearance letters, if applicable
(for conditions such as hypertension, diabetes, sleep apnea, heart conditions, or recent surgeries)

If Applicable

CPAP compliance report (minimum 90 days) if you have sleep apnea
Specialist documentation if you are followed by cardiology, endocrinology, neurology, or another provider
Previous DOT medical certificate, if available
If any recent surgery, medical clearance from specialist

Important Notes

Please arrive well hydrated, as a urine sample is required for all DOT physicals.
The urine test is for medical screening purposes only (not a drug test).
